Getting Started Is Easy
Call us at (530) 934-8700 during regular business hours, Monday through Friday, 8:00 AM to 5:00 PM. We'll verify your insurance, schedule your first appointment, and send over new patient paperwork to complete before your visit. We can't wait to meet your family.
What to Bring to Your First Visit
Your child's insurance card, a photo ID for the parent or guardian, completed new patient paperwork, any previous medical records or immunization history, and a list of current medications.
